NOMINATION FORMS AWARDS CRITERIA

The LUNA 2007 Awards Criteria is provided in six categories: `
PART I:             BUSINESS ADVOCATE OF THE YEAR
PART II:            ESTABLISHED FEMALE CONSTRUCTION FIRM OF THE YEAR
PART III:           ESTABLISHED FEMALE SERVICE FIRM OF THE YEAR
PART IIII:          HCA RISING FIRM OF THE YEAR
PART IV:           HCA OUTSTANDING FEMALE CONSTRUCTION/SERVICE WORKER OF THE YEAR
PART VI:           OUTSTANDING ADMINISTRATIVE PROFESSIONAL OF THE YEAR

(1)   Complete the appropriate nomination form (cover page) for the category in which the nominee is being submitted.
(2)   Answer each question in the nomination criteria on a separate sheet. The overall package should not exceed 12 pages.
(3)   Forward completed nomination package to:

Nominations can be e-mailed to make sure to confirm email has been received by 5:00pm on Friday June 15. Please note that nominees should be able to attend the Luna Awards event on June 28, 2007 at the DFW Airport Hyatt Regency Hotel. Please submit an original and two copies of the nomination package, including a one page narrative on the company for the Program and send a high-resolution company logo for use in materials related to the Luna Awards.
